Here's a picture of a 4-wheel ATV which is less likely to turn over, but they can have rollover accidents. The patient in the top right picture had a tire roll over his foot which created an avulsion injury. The other patient had an ATV roll over her leg. You can see she has abrasions to the leg below the knee and the thigh injury which has been excised.

Treatment for these patients requires trauma admission. The extremities are elevated, they are given analgesics and xrays are done to rule out fractures. They are given hydrotherapy and cleansing, the same way we would treat a burn, and silvadene is used. There are other topicals that can be applied, such as EZ-Derm or Xeroform, to temporize, but in general these have to be excised and either treated with a VAC sponge, an allograft temporarily and finally a skin graft.
