This is a young man with multisite disease who actually presented with dissecting cellulitis of the scalp. He was in the ICU with both gram positive and gram negative sepsis. We actually had to excise all the skin right down to the periosteum of the calvarium and then he was grafted. You can see the final appearance of the mesh graft on his scalp.
Unfortunately, he now has pilonidal cleft and inguinal disease which has been very difficult for him. Interestingly, he also had acne so I sent him to dermatology. They started him on Accutane and it has really made a big difference in helping to clear up his hidradenitis.
I was initially surprised by that, but there is some evidence to suggest that it may help.
