Antibiotics may be used in the mother during labor for premature labor, premature or prolonged rupture of membranes or fever or other signs of chorioamnionitis. Antibiotics cross the placenta and can achieve significant concentrations in the fetus. Since this is basically treatment of the fetus in utero, if the infant is infected and the organism is susceptible to the antibiotic, the baby's cultures may be falsely negative. Whether to continue antibiotics will depend on the same factors as previously discussed.

This situation most often occurs in the pregnancy complicated by GBS carriage. A typical algorithm for management of this clinical scenario is shown here. The management algorithm currently in use at this hospital is shown here.
