The subsequent management of the baby with negative blood cultures, done for signs of sepsis, becomes a judgment call on the part of the treating physician. Factors to consider in making a decision whether to stop or continue antibiotics include maternal history, maternal antibiotics, initial appearance of the baby, initial labs including WBC/diff, CRP, bacterial antigen tests and clinical course of the baby while on antibiotics.

Also, it must be recognized that occasionally cultures will not identify a baby who does indeed have a significant bacterial infection. One study showed that 18% of babies with unequivocal infection at autopsy had negative premortem blood cultures.
